New patient?

Are you new to Prime Surgery Center? Here are a few things you may want to know about our appointment:

New registration

As a new patient, you need to first register with us

  • You may register through our online portal
  • You may directly walk into our clinic and fill out the registration form
  • You may download and print the patient registration form, fill it out, and bring it with you to our clinic
Essential documents

You must bring a few papers with you when you first visit us. These include:

  • Health insurance card (if any)
  • Patient registration form (if you didn’t submit it online)
  • A paper mentioning all the medications you are currently taking and any specific mention of health condition
  • An identity proof like a driver’s license
Appointment cancellation

After booking an appointment with us, if you can no longer make it to the scheduled day, we request you to inform us about the same. It is best if you can let us know about your appointment cancellation within 48 hours of the appointment. It helps us schedule the appointment for other patients in need.

You must bring an ID (such as a Drivers License) and your current unexpired insurance or Third Party Payer card to your surgery center appointment.
